Let's use an employer who has 10 employees. This will work for any number. We are going to further assume that one employee has a pre-existing condition that requires group and another wants full maternity coverage found in group only. Eight of the employees will obtain a high deductible HSA (Health Savings Account) plan with a $2500 deductible. The other two will use our group health insurance plan that does not have any participation requirements. The individual plan can be list billed (a single bill for all the plans goes to the employer and is paid with a company check) and the employer will use after tax money from the employee to pay for the low cost plan. The group plan participants will also pay their plan from after tax money or, if they choose, the employer can use pre-tax money because it is group insurance. Now, the employer, using the Premium Only Section 125 plan can make contributions to the HSA accounts or pay a portion of the group plan. So, he might want to contribute $100 a month for each employee. This will give the employee $1200 a year in their health savings accounts. Over time it will build and unless they have some extraordinary expenses, they will not have to reach into their pockets anymore. The arrangement might go something like this: The employee covers his or her first $500 of expenses from the HSA health plan. The next $2,000 are covered by the employer and then the plan kicks in.
